OVERALLRESPONSIBILITY
The Clinical Case Manageris responsible for managing a clinical caseload, intake, and assessment ofneeds, developing individualized support plans, and implementing andcoordinating program plans for participants.Also included is counseling participants, organizing their activities,and working in cooperation with senior staff and others to achieve program andspecial project goals. The Clinical Case Manager also develops serviceproposals for funding, seeks opportunities to fill service gaps, and providestraining for staff on new and existing services.
