Job Summary
The Care Coordination Support Specialist / Patient Care will provide administrative and patient support to the Care Coordination team to help ensure efficient delivery of patient care services. This role involves assisting with appointment scheduling, patient follow-ups, documentation, and coordination of resources as directed by licensed Care Coordination staff. The position requires strong communication, organization, and interpersonal skills to effectively support patients, families, and healthcare providers.
Key Responsibilities
- Provide administrative assistance to patients and Care Coordination staff in supporting patient care plans
- Assist in scheduling patient appointments and coordinating care services as directed by Care Managers
- Help connect patients and families with appropriate community resources and healthcare services
- Maintain accurate and detailed progress notes for patient interactions
