Care Partner Reliever Community Services - Adelaide Hills & Murray Mallee Region

The Care Partner Reliever will oversee and assess the services provided, monitoring outcomes against each individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals.

Offered as a permanent full-time opportunity based from our Murray Bridge Community Services office, with some travel to Littlehampton and across the Adelaide Hills and Murray Mallee region as required.

The Care Partner Reliever will work across the Support at Home program portfolios providing additional Care Partner support at site. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes clear and respectful communication, active listening, and providing timely updates.
