Case Manager

Schedule: Mon- Fri 9am-5pm

Location: Brooklyn, NY 11203

Pay: $24-$26/hour

Responsibilities Include:

• Provide case management services to clients, including assessment, planning, and coordination of services via use of EBP, TIC, MI


• Assist clients to achieve housing and recovery goals


• Conduct psychiatric evaluations


• Perform other related duties, as needed.

Job Requirements:

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.


• 2+ years of work experience in case management, social services, or supportive housing.


• Strong communication skills, both verbal and written.


• Ability to work independently and as part of a team.


• Knowledge of supportive housing programs and community resources is a plus.
