Overview

You work in a rehabilitative assessment unit as part of a multidisciplinary team that includes a doctor, charge nurse, nurse, practical nurse, social counselor, physiotherapist, and occupational therapist. Together, you assess clients' functional capacity and service needs, and support their independent living at home.

Duties and Responsibilities

• Plan, implement, and evaluate nursing care and services that promote clients' rehabilitation as the charge nurse.
• Act as an expert in nursing and ensure the implementation of individual care and rehabilitation plans.
• Plan, implement, and evaluate medication management in collaboration with the unit's doctor, serving as the medication responsible person according to the medication plan.
• Monitor and evaluate clients' overall condition and rehabilitation progress using measures such as RAI and participate in multidisciplinary assessment work.
