Clinical Care Partner - Support At Home

We are seeking an experienced Registered Nurse with a strong Care Advisory background within Home Care Packages (HCP) and experience supporting the transition to Support at Home (SAH) to join a welcoming and supportive team.

This is a full-time, 3-month locum position based Monday to Friday in the Brunswick office, with an immediate start available. There is also the potential for the role to transition into a permanent position for the right candidate.
