Description

Reporting to the HCC Nurse Manager, the Home and Community Care (HCC)/Public Health Nurse develops, coordinates and implements in-home nursing services for HCC clients residing in KO communities, and works with a team of Community Personal Support Workers (PSWs), Healthcare Professionals, KO team and Community Leadership. This individual also directly provides in-home, client-centered nursing services to clients. Additionally, as a Public Health Nurse, the nurse will provide health care services to support the needs of school age children, youth, and community. The PH nurse will help to support broader community health and wellbeing that includes immunizations, rapid testing, case tracing and case management.
