Job Description
Position Overview
We are seeking an experienced Home Health Registered Nurse to provide skilled, patient centered care to individuals in their homes throughout the Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, and Framingham areas. This role follows LHC home health RN standards and focuses on comprehensive assessments, OASIS documentation, care coordination, and patient education to support high quality outcomes in the home setting.
Schedule & Work Details
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours, with the opportunity to work up to 40 hours
- Productivity Expectations:
- 26 points in 32 hours
- 33 points in 40 hours
