Well established home health agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include the Attleboro/Norton/Mansfield, MA area.

• Monday-Friday, 40 hours/week
• Weekend rotation
• Highly competitive pay rate plus IRS mileage reimbursement
• 13-week contract

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
