Location: Greater Western Sydney | Salary: $70,000–$85,000 + Super
Crescent Care’s Housing & Homelessness Assistance program is seeking a dedicated Case Worker to provide end-to-end support to individuals and families at risk of homelessness. You will work closely with clients to develop sustainable housing solutions and improve long-term outcomes.
Key Responsibilities
- Conduct housing assessments and develop case management plans.
- Provide tenancy support, budgeting advice, and assistance securing long-term accommodation.
- Advocate on behalf of clients with housing providers and government agencies.
- Facilitate access to mental health, employment, and financial counselling services.
- Prepare detailed case notes and compliance reports.
