Position Summary
The Life Redesign Coach provides trauma-informed, person-centered, and culturally responsive coaching support to individuals participating in Hamilton Urban Core Community Health Centre's Residential Treatment and Recovery Program as well as the main HART Hub site.
The role supports individuals navigating recovery, stabilization, community reintegration, housing transitions, and life redesign following experiences of substance use, mental health challenges, homelessness, housing instability, poverty, and other complex psychosocial needs...
