As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will work onsite at our Conviva Lake Okeechobee Center in Okeechobee Florida. You will report to our Associate Medical Director.
