Nexus PCN is seeking a qualified Clinical Pharmacist to join our supportive multi-disciplinary team in a patient-facing role. You will work alongside another experienced pharmacist, ensuring a collaborative environment where your expertise can thrive. As an Independent Prescriber , you will play a key part in managing chronic diseases, reducing medication waste, and supporting our prescribing agenda. Key responsibilities include conducting Structured Medication Reviews (SMRs) for patients with complex polypharmacy, addressing medication queries, enhancing prescription processes, and leading clinical audits to improve patient safety. You will work to optimise medication use and ensure safe, effective medicine management across the Network.
