Grande Prairie Regional Hospital is looking for a Pharmacist who is interested in a role with a strong clinical focus and regular involvement in direct patient care. You will work closely with patients, prescribers, nurses, and other members of the healthcare team to assess medication therapy, resolve drug-related problems, and support safe, practical treatment plans. This role offers a broad mix of acute and chronic care, with opportunities to use your clinical judgment every day. The work includes medication therapy assessment and monitoring, patient education, discharge and transition planning, and collaboration with the care team when medication regimens need to be adjusted.
