Position Overview

The Home Health Registered Nurse provides skilled nursing care to patients in their homes in accordance with physician orders, established plans of care, and agency policies. This role requires strong clinical judgment, efficient time management, and the ability to work independently while meeting productivity and quality standards. The RN collaborates closely with physicians and the interdisciplinary team to promote positive patient outcomes and continuity of care.

Schedule & Work Details

• Schedule: Monday through Friday


• Hours: 8:00 am to 4:30 pm


• Weekly Hour Guarantee: 36 hours


• Productivity Expectation: 30 points per week


• Coverage Areas: Portage, Summit, and Medina Counties


• On Call Requirement: One weekday per week and one weekend per month


• Mileage Reimbursement: No


• Pay Rate: $51 to $53 per hour
