Job Description
Assignment Details
- Work hours: 40 hours/week with 80% home visits & 20% administrative tasks to update PCA assessments & organize notes.
- Shift window: Visits will be between 7:00 AM – 6:00 PM
- Duration:
- Initially 3 months
- Work available for up to 6 months
- Equipment provided:
- iPhone
- Tablet
- Other required tech devices for assessments
Role Overview:
- Home visit based role (10-20% remote desk job to organize cases & notes)
- Goal is to complete as many in home PCA assessments as possible
- Responsibilities include:
- Outreach to members
- Visiting members’ homes
- Completing system based PCA assessments using the PCA tool
