Social Worker - 0.2 FTE

We are currently recruiting one part-time Social Worker to join our Interprofessional Primary Care team.

The Primary Care Social Worker is part of an interprofessional health team, providing biopsychosocial assessments and interventions for diverse populations. This role focuses on improving overall well‑being by addressing social determinants of health, mental health concerns, and system navigation challenges. The Social Worker collaborates closely with patients, primary care providers, and community partners to enhance access to comprehensive, patient‑centered care.

A Successful Candidate in This Role Will Have

• Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred.
• Membership in good standing with the Ontario College of Social Work and Social Service Workers.
• Possess professional liability insurance.
