Our client in Telford is looking for an experienced Adult Social Worker to join their Hospital Social Work Team.

What will your responsibilities be?
In this role, you will conduct Enablement Reviews and Care Act Assessments to support individuals in regaining their independence and complete assessments as required by the Care Act. You will assess individuals’ capacity to make decisions and conduct Best Interests assessments when necessary under the Mental Capacity Act. Utilizing an asset-based approach to social work, you will focus on the strengths and abilities of individuals to promote their well-being. You will complete Continuing Healthcare (CHC) checklists to determine eligibility for NHS continuing healthcare funding.
