Position Overview: We are seeking a dedicated and compassionate Care Management Specialist to join our team. In this role, you will play a pivotal role in ensuring the effective coordination and delivery of social services, discharge planning, and care management for patients. You will actively participate in patient transitions, interdisciplinary team discussions, and quality improvement initiatives to promote the best outcomes for patients and their families.
