Social Worker The Veterans Health Administration is seeking a full-time Social Worker to support adult and geriatric Veterans enrolled in the Home Based Primary Care (HBPC) Program at the Finger Lakes Healthcare System. This position provides case management, clinical treatment, advocacy, psychosocial assessment, treatment planning, and coordination with VA and community-based service providers.
